Required Documentation for Medicaid Applications

Documents you will typically need to submit when applying for Medicaid

If you’re getting ready to begin the process of applying for Medicaid coverage for yourself or your aging loved one, collecting and submitting the required documents is a significant task.

Below is a general outline of what you can expect Medicaid to require when you apply. Of course, additional documentation may be requested based on the specific state you are applying in and your personal and financial circumstances.

Categories of Documentation

At least one document should be provided to support each of the following categories.

✔ Proof of Age
Age-proving document, such as a US Passport, Birth certificate, or Driver’s license.

✔ Proof of Citizenship
Citizenship document, such as a US Passport, Birth Certificate, or Naturalization Papers.

✔ Identity
Identity proof, such as a US Passport, Photo License, or US Military ID.

✔ Marital Status
Document proving marital status, such as a Marriage Certificate, Separation Papers, or Divorce Decree.

✔Financial Resources
All data applicable to Resources owned in the last 5 years should be submitted.
This includes bank statements of both Checking and Savings accounts, Money Market accounts, Certificates of Deposit, and other applicable documents.

✔Income
Documents supporting income level, such as Recent Paystubs, Social Security Award letter, Railroad Retirement Reward, as well as any other applicable income documentation.

✔ Miscellaneous
Other documents that should be submitted include copies of the front and back of your social security and Medicare Cards, Additional Insurance or Rx Card, Insurance Premium Bill or Payment Coupon, POAs, and Guardianship Documentation.

Proof of Living Expenses

If the Medicaid recipient is placed in a nursing facility, but his/her spouse remains living in the community, the following documents supporting the spouse’s living expenses will be taken into account when applying for Medicaid:

  • ● All Income Verifications (SS Award Letter, Pension Stubs, etc.)
  • ● Spouse’s ID & Cards
  • ● Spouse’s ID & Cards
  • ● Electric Bills
  • ● Real Estate Tax Bills
  • ● Gas/Oil Bills
  • ● Mortgage Statements
  • ● Rent Receipts
  • ● Water/Sewer bills
  • ● Home/Renter’s Insurance


A copy of the applicable documents above should be submitted to support these expenses.

Compiling the Documents
